The Brandon Pediatric Clinic provides comprehensive primary care for military children and adolescents. Our staff are specifically trained to evaluate and manage the full spectrum of the medical needs of children and young adults. Our clinic includes four providers, three nurses, and six medical technicians who provide care for 3,700 children.

We request that families complete the below Pediatric Health Form once on each child enrolled to our clinic, and bring with you to your child's next appointment.

Below are the questionnaires for well child visits. Please note that the forms for age seven and older also include the Florida sports physical form. If you do not need this completed, please just fill out Part 2 for our own information. Also, if you are coming in for a 9-month, 18-month, or 24-month visit, we ask that you complete a detailed developmental questionnaire called "Ages and Stages." The form is copyrighted and cannot be posted online; please contact us by phone or MiCare to have this sent to you.
